To have a meaningful impact in reducing the cost of college for students, we strongly recommend establishing new scholarships at $1000 or higher. You may enter special instructions in the comment field below. If you wish to provide multiple scholarships, enter the total amount of funding, and note the details in the "Special Instructions" field below.
How should the scholarship be listed in our records? Examples: John Smith Memorial Scholarship, Company A Scholarship, New Haven Teacher's Association Scholarship, Lions Club Scholarship. If, this scholarship is to be categorized within a group (PASE, DAAA, MVTF, etc.), please include that at the beginning of the name.

We strongly recommend using objective criteria, as those types of qualifications work well with our AwardSpring scholarship system. If you aren't sure how to make your criteria objective, just enter your thoughts and we will assist.
Who will select the scholarship recipient?
If not specified on this form, scholarship recipients will be selected by the Foundation board. If you will have a team of scholarship reviewers, we will need the name and email address for each person prior to January 31. You may include that information in the instructions, if you know it now.
Certification of Impartiality*
The selection process must be impartial. If any reviewer has a close personal relationship with an applicant, that person must abstain from the review process for that year. This may happen on occasion, and Foundation board members would be happy to assist you with the review process.
Payment - New Scholarships*
Payment for first-year scholarships is to be provided in full prior to the start of the application payment for students. For the Class of 2026, payments are due on or before December 15, 2025.
